Design Professional’s Lien

A four-page form set for documenting a California lien claim by a qualifying architect, landscape architect, professional engineer, or land surveyor for unpaid design services provided before construction begins.

The form includes sections for the design professional’s license, fees, prior payments, lien amount, property, record owner, building permit or governmental approval, services provided, written agreement, and payment demand. It also includes a California notarial acknowledgment, important notices, and a pre-recording eligibility checklist.
